Student Records Form
Mt. Vernon Community School Corporation maintains and releases student education records in accordance with applicable federal and state laws, including the Family Educational Rights and Privacy Act (FERPA), Indiana Code, and Board Policy E175. Education records include official records directly related to a student and maintained by the Corporation. Records will be released only to authorized educational institutions or as otherwise permitted by law. Student medical records resulting from school-based or contracted treatment services are confidential and are not included in the student's permanent educational record or released as part of this request.
Please keep in mind during summer MVCSC School Offices are not open for request. Records will be sent in a timely manner upon return.
Public Records Form
This form is used to submit requests for public records to Mt. Vernon Community School Corporation under Indiana’s Access to Public Records Act, Indiana Code § 5-14-3. In accordance with Indiana law, including recent legislative allowances for electronic public records request portals, this form is used to receive, verify, track, and process APRA requests. Requests should identify the records sought with reasonable particularity.
Records or portions of records may be withheld or redacted when required or permitted by law, including student education records protected by FERPA, confidential personnel information, attorney-client privileged communications, and other information exempt from disclosure.
Submission of this form does not guarantee that all requested records are disclosable. Mt. Vernon Community School Corporation will review and respond to requests in accordance with Indiana law.
